Sustainable donors provide essential committed support on a monthly basis. And becoming an IHS sustainable donor is easy! Simply fill out our form and submit online, or print out and mail it in to the address listed below. If you would like IHS to deduct your donation from your checking account, please send a signed voided check to the below address.

Please remember:

Gifts can be made from a checking/savings account or credit card (Visa, MasterCard, or American Express).
A minimum of $10.00 per month is required.
Choose which day of the month you want the funds to be charged to your account.
- We have four dates available: 1^st, 5^th, 15^th, or 20^th of each month, unless you’re using an American Express card, in which case your gift will be charged on the 20th of each month.
