Community Based Case Management Program (CBCM)

The Community Based Case Management (CBCM) program serves homeless individuals who are challenged with chronic mental illness. Participants must be:

  1. Declared eligible by the State of Hawaii, Department of Health, Adult Mental Health Division [for those who are uninsured or have QEXA insurance coverage] OR
  2. Declared eligible by their IHS CBCM participating Quest plan (for those with Quest insurance, e.g. AlohaCare or United Behavioral Health.)

The CBCM team currently serves up to 125 homeless individuals (men or women) at any given time. The initial goal of this program is to help clients achieve stability with obtaining basic needs such as safe shelter, financial assistance, and medical insurance. Help offered includes symptom and medication management, pre-employment and employment, social support, skill building, access to specialized services, and enhanced capacity to function independently in the community. Supportive counseling and assistance with linkage to community resources are also key. Provided services are guided by principles of instilling hope and expanding competency, which involve the participant in developing his/her personal plan for recovery.

Eligibility:

  • If uninsured or a QEXA or Medicare member, must be referred by AMHD through ACCESS or by another provider.
  • If insured, must have AlohaCare Quest with BHPP benefits or United Behavioral Health Quest insurance (with behavioral health case management benefit.)
  • Must have eligible Axis I diagnosis.
  • Must be 18 years and older.
  • Must be homeless or at risk of homelessness.
